Twerking Radio guest Aldo Hernandez (download his episode here) was kind enough to share a copy of those forms DJs used to fill out when they belonged to record pools. As I understand it, vinyl DJs had access to new records but were asked to keep track of what they were playing so record companies could get a sense of what was being played and by whom and where.
This particular form comes from Aldo's legendary Meat / Squirt Farm party over in the meatpacking district of New York City when there were still tons of tranny hookers and meat lockers located on 14th st. and Washington St. Hell yes to Orbital and Lords of Acid. Thanks Aldo!
